Forgot how much I paid last time but $700 shouldn't be too far off. Also, it depends on the thickness.

$700 is reasonable price.  My friend ordered one with curved edges on the top side and it cost HK$1000 two years ago

Seamless is usually far more expensive.

2mm thickness is no good. You should ask for 5mm thickness for arcylic material

Yes! It is better to have seamless for left, top and right sides.  Front and back sides to be glued on.

Yes, they have covers for all their models and with dimensions as below link:

The $1500 is the price I got quoted from one of the dealers.  However, i dont see the value of ordering direct from them and the wait maybe months whereas the local ones took only 1 week.
